Please note: The only paperwork you need to submit with your child's application is their current Immunisation History Statement and any court order affecting them. These documents are legally required.

Please do NOT attach any birth certificate, Medicare card (we do need the number, just not the card) or Health Care Card. We are aware the form asks for these, however we have been unable to get the software company to remove those fields.
